The New York State Education Department requires a physical health examination for public school students entering the school district for the first time (new entrants) and students entering grades Pre-K or K, 1, 3, 5, 7, 9 and 11, as well as annually for interscholastic sports; for working papers as needed; or as required by the Committee on Special Education (CSE) or Committee on Pre-School Special education (CPSE).As of January 31, 2021, school physicals must be on the Required NYS School Health Examination Form found here:
