If you do not want your children to be included in photographs, articles, or other District communications for the current or upcoming school year, fill out the following form and return it to your child's school:
If you returned the above form and wish to change your preference or wish for your child to be included in District communications for a specific purpose only (e.g. a one-time article about a specific award or distinction or a student slideshow at an end-of-year student assembly), please fill out this permission form
and return it to your child's school. Note: You only have to fill out the permission form if you previously filled out the Exclusion from Student Photographs, Interviews and Recordings form.
