    ***FOR SEPTEMBER 2021:  All 7th graders in the Fall 2021, will need to receive a meningitis vaccine.  Please contact your health care provider to see if your child has had this, or if you need to make an appointment to receive one. Proof of this vaccine will be required for entry into 7th grade in September 2021.  Further information can be found here://www.health.ny.gov/diseases/communicable/meningococcal/
     
    All students are required to have health care provider signed proof of the following immunizations in order to attend school:
     
     5th grade
    • 3 doses of the Polio vaccine 
    • 3 doses of the Hepatitis B vaccine
    • 3-5 doses of the DTP (Diphtheria/Tetanus/Pertussis) vaccine
    • 2 doses of the MMR (Measles/Mumps/ Rubella) vaccine
    • 1 dose of the Varicella (Chickenpox) vaccine
    6th and 7th grade 
    • 3 or 4 doses of the Polio vaccine **(4 doses if all were given before age 4 OR 3 doses with the 3rd dose at 4 years or age or older)
    • 3 doses of the Hepatitis B vaccine
    • 3 doses of the DTP/DTaP vaccine
    • 2 doses of the MMR vaccine
    • 2 doses of the Varicella (Chickenpox) vaccine
    • 1 dose of the Tdap (Boostrix or Adacel) vaccine, which is usually given at 11 years of age
    • 1 dose of the Meningococcal vaccine for 7th grade
     
    There is a 4 page form that must be submitted for religious exemptions (see the school nurse for this form).
     
    Physician signed proof for medical exemptions for immunizations must be submitted annually.
     
    The grace period allowed by NYS law for new entrants to a school/school district are as follows: 14 days for in-state, and 30 days for out-of-state or out-of-country from the time that the child enters the new school/school district.   Failure to provide or obtain the required immunizations will result in exclusion from school per NYS Public Health Law 2164.
